Guidance

Registration (Forms)

All registrations require a Parent/Guardian to accompany the student.

For students that are already attending an Orange County Public School and now are registering with Edgewater High School the following forms are needed:

NOTE: THIS APPLIES FOR STUDENTS WITH A TRANSFER

• Birth Certificate
• Social Security Card
• Proof of Residency – Lease/Rental Agreement or Homestead Exemption Card. (NO BILLS PLEASE)
• Special Education Students (ESE) a copy of the current IEP
• Guardianship papers (if applies)

For students that are not coming from an Orange County Public School the following documents are required:

• Birth Certificate
• Social Security Card
• Proof of immunization on a Blue From 680 (Health Dept 407-836-2600)
• Proof of Residency -- Lease/Rental Agreement or Homestead Exemption Card. (NO BILLS PLEASE) For non-residents of Florida Declaration of Domicile.
• Grades (Report Card), credits received and/or withdrawal grades.
• Special Education Students (ESE) a copy of the current IEP
• Legal Guardianship papers (if applies).
